Can a Real-World Problem Become a Research Topic?

A real-world problem can be an excellent starting point for research, but the problem itself is not yet a study. You need to establish what is happening, investigate what is already known, decide what remains uncertain, and formulate a question that research can actually answer.

01 · The Question

Can You Start Research With a Problem You See Outside Academia?

A clinic has a service that patients rarely use. A manufacturer keeps encountering the same production delay. A school introduces a support program, but participation remains low. A local government provides an online service that residents continue to access in person. An organization invests in new software, yet employees maintain their old workarounds.

These are real problems, but are they research topics?

Potentially, yes. Research does not have to begin with an abstract academic debate. Practical problems can generate important research when there is something about them that needs systematic investigation. The critical step is moving from “something is going wrong” to a defensible question about what needs to be understood, explained, evaluated, compared, or tested.

A Practical Problem Can Become Research, but It Needs Translation

In Brief

Yes. A real-world problem can become a research topic when it contains an important uncertainty or question that systematic investigation can help answer.

The practical problem is the starting situation, not automatically the research problem or research question. Before designing a study, establish what is actually happening, examine existing knowledge, involve relevant stakeholders when appropriate, identify what remains uncertain, and formulate a question that is significant, ethical, feasible, and answerable.

03 · What You Need to Know

How a Practical Problem Becomes a Researchable Problem

Start by Describing the Problem Without Explaining It Away

A useful first move is surprisingly simple: describe the situation before deciding why it exists.

Suppose a community health program offers free appointments, but many eligible people do not attend. “Eligible people are not using the service at the expected rate” describes a discrepancy that may warrant investigation. “People do not attend because they do not understand the benefits” is already a causal explanation.

That explanation might eventually prove correct. It might also be wrong. Transportation, inconvenient scheduling, distrust, stigma, competing responsibilities, poor communication, eligibility confusion, previous experiences with the service, or other factors could be involved.

WHO's implementation research materials explicitly approach practical challenges this way: identify bottlenecks and relevant stakeholders, formulate research questions, conduct research, and use the findings to inform action. The toolkit's problem-analysis guidance asks researchers to examine discrepancies between actual and intended situations and consider multiple possible explanations rather than assuming a single cause.

A Real-World Problem and a Research Problem Are Related but Not Identical

A practical problem concerns a condition in the world that someone may want to change: low uptake, excessive delays, unequal access, recurring failure, poor retention, unexpected behavior, environmental degradation, inefficient processes, or another consequential situation.

A research problem identifies what about that situation needs investigation. Perhaps the causes are unclear. Perhaps an intervention works in one setting but not another. Perhaps stakeholders disagree about what the problem actually is. Perhaps a proposed solution has never been adequately evaluated.

Practical problem Something in the world is happening, not happening, or producing outcomes that stakeholders consider important.
Research problem Something relevant to understanding or addressing that situation remains uncertain, insufficiently explained, or inadequately tested.
Research question A focused question states what the investigation will try to find out about that uncertainty.

This is one reason it helps to distinguish the topic, research problem, and research question. A practical concern can motivate all three without being interchangeable with them.

First Establish That the Problem Is Actually a Problem

Before investing months or years in explaining a problem, check whether your description of it is supported.

How large is the discrepancy between what is happening and what was expected? How many people or processes are affected? How serious are the consequences? Has the situation persisted long enough to distinguish it from ordinary variation? Who considers it a problem, and do other stakeholders see it differently?

WHO's implementation research guidance includes comparable considerations when prioritizing problems and questions, including how widespread or severe a problem is, who is affected, who considers it a problem, its burden, and whether the issue or question has already been researched.

The evidence you need depends on the subject. Administrative records, monitoring data, previous evaluations, interviews, observations, published studies, policy documents, or other sources may help establish the situation before you formulate a larger research project.

Ask Who Gets to Define the Problem

Real-world problems often involve multiple groups whose priorities do not perfectly align.

Hospital administrators may define a problem as excessive waiting time. Clinicians may see understaffing and workflow design as the central issue. Patients may care most about communication, uncertainty, cost, or whether the visit ultimately resolves their concern.

This does not mean every research project must satisfy every stakeholder equally. It does mean that problem definition should not automatically inherit the perspective of whoever has the most institutional authority.

Implementation research offers a useful illustration. TDR describes implementation research as demand-driven, with questions framed from needs identified by relevant stakeholders and implementers working within the health system. Research on implementation also emphasizes collaboration between researchers, decision-makers, frontline workers, patients, and other affected groups when translating practical challenges into researchable questions.

Find Out Whether the Answer Is Already Known

Not every operational difficulty requires new research. Sometimes the necessary knowledge already exists and the real challenge is applying it.

Imagine an organization discovering that staff routinely make a particular procedural error. If extensive evidence already identifies the cause and a well-supported corrective process exists, another exploratory study may add little. The organization may need implementation, training, resources, evaluation, or quality improvement rather than a new investigation of the same basic question.

Conversely, a proven intervention may work under controlled conditions but encounter barriers when implemented in routine settings. That itself can generate research questions about adoption, feasibility, acceptability, reach, context, sustainability, or implementation strategies. Implementation research specifically addresses questions about how evidence-informed interventions work in real-world settings and why implementation succeeds or fails.

Separate the Problem From Your Preferred Solution

People who encounter practical problems often arrive with solutions already in mind. A manager sees low productivity and proposes monitoring software. A university sees poor student attendance and proposes stricter attendance requirements. A city sees traffic congestion and proposes a particular transport intervention.

That can create a subtle research mistake: defining the study so that it evaluates only whether the preferred solution should be implemented rather than investigating whether the diagnosis behind that solution is sound.

Try writing the problem without mentioning the solution. If you cannot do so, you may have framed the situation too narrowly.

Watch Out

“How can we prove that intervention X will solve the problem?” is not an open research question. The wording has already assumed the intervention works. A defensible study should permit findings that challenge the proposed explanation or solution.

Decide What Kind of Knowledge Would Actually Help

A single practical problem can generate very different research questions because researchers may need different kinds of knowledge.

If You Need To... You Might Ask...
Describe What is happening, to whom, where, and how often?
Explore How do affected people experience or understand the problem?
Explain What factors or mechanisms may account for the observed pattern?
Compare How does the problem differ across groups, settings, conditions, or approaches?
Evaluate Does an intervention produce the intended outcome, and under what conditions?
Understand implementation What barriers and facilitators affect adoption, delivery, reach, or sustainability?

WHO's practical guide to implementation research similarly distinguishes questions intended to describe, explore, explain, influence, or predict. The purpose of the question helps determine what methods will be appropriate.

Do Not Make the Research Question Bigger Than the Problem Requires

Real-world problems can be enormous. “Food insecurity,” “traffic congestion,” “teacher shortages,” “misinformation,” and “climate adaptation” describe domains containing countless interacting problems.

A research project does not need to solve the entire domain. It needs to investigate a defensible part of it well.

Suppose a university experiences high dropout rates. A project attempting to discover “all causes of university dropout and how to eliminate them” is unlikely to be manageable. A more focused study might investigate how a particular group of students describes the factors influencing withdrawal during a defined stage of study, or evaluate whether a particular intervention affects retention under specified conditions.

The problem should guide how you narrow the research topic. Narrowing is useful when it creates a study that can produce meaningful evidence, not when it merely adds arbitrary boundaries.

Feasibility Can Change the Question Without Making It Unimportant

You may identify an important problem but lack access to the evidence required to investigate your preferred question.

A researcher studying workplace discrimination may not be able to access confidential personnel records. A researcher interested in emergency response may be unable to manipulate operational conditions experimentally. A student may identify a national policy problem but have time and resources for only a small, well-defined study.

The correct response is not necessarily to abandon the problem. You may need to ask a different question that contributes useful knowledge within realistic constraints.

This is where the relationship between the problem and the data you can actually access becomes consequential. Data availability should influence feasibility without becoming the sole reason a question exists.

Research Does Not Have to Produce an Immediate Solution

A study can be useful without ending with a ready-to-implement intervention. Sometimes the most valuable contribution is accurately defining the problem, identifying previously misunderstood barriers, testing an assumption, explaining variation, or establishing that a proposed solution does not work as expected.

Applied research is still research. The fact that stakeholders want actionable results does not eliminate requirements for appropriate design, transparent limitations, ethical conduct, and conclusions that match the evidence.

Likewise, not every research finding will transfer directly into policy or practice. Implementation depends on context, resources, values, feasibility, stakeholder priorities, and the strength and applicability of the evidence.

A Practical Problem Can Lead to Several Legitimate Studies

There is rarely only one research question hidden inside a real-world problem. Different disciplines and stakeholders may ask different questions about the same situation.

A decline in public-transport use could generate research on passenger behavior, service reliability, urban design, pricing, accessibility, environmental consequences, communication, or policy. Those studies are not interchangeable even though they originate from the same practical concern.

This is why starting with a problem can be productive without determining your study in advance. The problem establishes a reason to investigate; research development determines which part of that problem your project can credibly address.

04 · A Practical Example

Turning Missed Appointments Into a Researchable Study

Hypothetical Example

A Community Clinic Has a Persistent Appointment Problem

Imagine a community clinic notices that a substantial number of scheduled follow-up appointments are missed. Staff members believe reminder messages should solve the problem, but the clinic has not systematically investigated why patients miss appointments.

Observed problem The clinic's records show a persistent discrepancy between scheduled follow-up appointments and completed visits.
Initial assumptions Staff suggest forgetfulness, but other plausible explanations include transportation, work schedules, caregiving responsibilities, cost, communication problems, dissatisfaction, or difficulty rescheduling.
Existing knowledge The researcher reviews relevant literature and local evidence to determine what is already known about missed appointments and which explanations remain uncertain in this setting.
Research problem The clinic lacks sufficient evidence about the factors influencing missed follow-up appointments among the population it serves, limiting its ability to choose an appropriate response.
Candidate question What factors do patients who have missed follow-up appointments identify as influencing their ability or decision to attend?
Possible next study If the first investigation identifies a modifiable barrier and a plausible intervention, a subsequent study could evaluate whether addressing that barrier improves attendance.

Notice that the researcher did not begin by testing reminder messages simply because staff members preferred them. The first study addresses the uncertainty underneath the practical problem. Depending on what is already known and what local evidence shows, another design might be more appropriate, but the principle remains the same: investigate the question the problem actually creates.

The example also illustrates why research may proceed in stages. Understanding a problem, developing an intervention, evaluating effectiveness, and studying implementation can require different questions rather than one oversized project pretending to do everything.

05 · What Researchers Often Get Wrong

Common Mistakes When Turning Practical Problems Into Research

Misconception

If a Problem Is Serious, Is It Automatically Researchable?

No. Importance and researchability are different. A problem may be urgent while your proposed question is too vague, ethically unacceptable, impossible to investigate with available evidence, or too large for one study. You need both a worthwhile problem and a credible way to investigate part of it.

Misconception

Do You Need to Discover a Problem Nobody Else Has Seen?

No. A well-known problem can still contain unanswered questions. Research may clarify mechanisms, compare approaches, examine context, test an intervention, investigate implementation, or produce evidence needed for a particular decision. Novelty does not require discovering an entirely new social or practical difficulty.

Misconception

If Practitioners Know the Problem, Do You Still Need the Literature?

Yes. Practical knowledge can reveal what is happening locally, while existing research can show how similar problems have been conceptualized, investigated, and addressed elsewhere. The literature may also reveal that your initial explanation has already been tested or that relevant evidence exists under terminology practitioners do not normally use.

Misconception

Does Applied Research Have to Solve the Problem?

No. A useful study may identify causes, characterize experiences, evaluate an intervention, explain why implementation fails, or rule out an assumed explanation. Research should produce defensible knowledge relevant to the problem; promising a complete solution may exceed what one study can establish.

Should the Person Who Owns the Problem Decide the Research Question?

Stakeholder priorities can be crucial, especially in applied and implementation research, but the question also needs methodological scrutiny. Different affected groups may define the problem differently, and researchers must consider existing evidence, feasibility, ethics, and what the proposed design can actually answer.

Misconception

If You Already Know the Solution, Is Research Just About Proving It Works?

No. If effectiveness is genuinely uncertain, evaluating a proposed intervention can be legitimate. But the design should allow the intervention to fail, produce unintended effects, work only under certain conditions, or reveal that the original diagnosis was incomplete.

06 · What This Means for You

Diagnose Before You Design the Study

If your research idea comes from a real-world problem, resist the urge to jump directly from problem to solution. First determine what you actually need research to establish.

A simple decision framework

If you know something undesirable is happening but do not know its extent
Begin by asking what evidence would establish the size, distribution, frequency, or consequences of the problem.
If the problem is established but its causes or mechanisms are uncertain
Develop a question that investigates plausible explanations without building your preferred explanation into the wording.
If affected people understand the problem differently
Consider whether exploring stakeholder experiences and perspectives is necessary before defining an intervention or evaluation.
If a solution already has strong evidence but is not working in practice
Consider questions about implementation barriers, facilitators, context, adoption, reach, fidelity, feasibility, acceptability, or sustainability rather than repeating the original effectiveness question.
If you already have a proposed intervention but its effects are uncertain
Determine what comparison, outcomes, timeframe, and design would be needed to evaluate it credibly.
If the ideal question requires evidence you cannot obtain
Reframe the project around a narrower question that still contributes useful knowledge instead of pretending the available data can answer more than they can.

WHO guidance on implementation research makes the same broad connection between problem definition and study design: the research question should be shaped by the problem and should, in turn, shape the research design. It should also be clear, specific, answerable, and capable of generating important information.

Watch Out

A sponsor, employer, community organization, or decision-maker may have a legitimate practical need while also preferring a particular result. Clarify the research question, decision-making roles, data access, analytical independence, reporting expectations, conflicts of interest, and whether unfavorable findings can be reported before committing to the project.

Check Whether a Real-World Problem Is Ready to Become Research

Before turning the problem into a study, check:
Describe what is happening separately from your assumptions about why it is happening.
Verify the existence, scale, persistence, and consequences of the problem using appropriate evidence where available.
Identify who is affected and whether different stakeholders define or prioritize the problem differently.
Search the relevant scholarly and professional literature to determine what is already known and what remains uncertain.
Ask whether new research is actually needed or whether existing evidence already supports an appropriate response.
Separate the problem from any solution that you or stakeholders already prefer.
Specify whether the study needs to describe, explore, explain, compare, evaluate, or investigate implementation.
Formulate a focused question that can be answered with a defensible research design.
Confirm that the necessary data, participants, expertise, time, resources, permissions, and ethical conditions are realistically available.

Questions About Researching Real-World Problems

What is the difference between a practical problem and a research problem?

A practical problem concerns a situation in the world that may require action. A research problem identifies something about that situation that needs systematic investigation. Research can inform action, but the practical problem and the knowledge problem are not identical.

Can a workplace problem become a thesis or dissertation topic?

Potentially. The project still needs scholarly significance appropriate to the degree, engagement with relevant literature, a focused research question, a defensible methodology, feasible access, and appropriate ethical arrangements. Employment relationships, confidential information, and power differences may create additional considerations.

Can a community problem become a research topic?

Yes. Community problems can generate important research questions, particularly when affected communities are appropriately involved in defining priorities and interpreting what matters. Researchers should avoid assuming that an outsider's definition of the problem automatically represents the community's priorities.

Does the research have to provide a solution?

No. Depending on what is currently unknown, a valuable study might define the problem more accurately, identify contributing factors, understand experiences, test assumptions, evaluate an intervention, or explain why an existing solution is difficult to implement.

What if the solution already exists but people are not using it?

That may create an implementation research question. Instead of asking again whether the intervention can work, you might investigate barriers and facilitators affecting adoption, reach, delivery, acceptability, feasibility, or sustainability in the relevant setting.

Do I need a research gap if I already have a real-world problem?

You still need to establish how your study relates to existing knowledge, but this does not necessarily mean finding a completely untouched subject. The contribution may involve resolving uncertainty, testing an explanation, evaluating an intervention, investigating implementation, examining context, or producing evidence needed for a consequential decision.

Can one real-world problem generate several research questions?

Yes. A practical problem can contain descriptive, explanatory, comparative, evaluative, methodological, and implementation questions. The challenge is selecting the question that is most important and feasible for your particular study rather than attempting to investigate every dimension at once.

Establish the problem before explaining it, listen to relevant stakeholders, examine what is already known, separate diagnosis from your preferred solution, and formulate a question that your evidence and methods can actually answer. The goal is not merely to study something that matters in practice, but to produce knowledge that helps people understand that problem more accurately.
