How Do You Decide Whether Recruitment Is Feasible Before Committing to the Main Study?

Recruitment is feasible when the full recruitment pathway can realistically produce the sample your study needs within the available population, sites, time, and resources. Testing feasibility requires more than showing that some participants can be enrolled.

01 · The Question

How Can You Know Whether Your Recruitment Plan Will Actually Deliver the Sample?

Your sample-size calculation says you need 300 participants. Your recruitment plan says you will approach students, patients, teachers, employees, households, or another target population until you reach 300.

There is an important gap between those two statements.

How many potentially eligible people are actually accessible? How many satisfy the eligibility criteria? How many can be contacted? How many agree to participate? How long does each stage take? What happens if the recruitment rate slows after the easiest participants have been enrolled?

Recruitment feasibility is about whether the recruitment system, not merely the target number, can realistically produce the sample required by the main study within its available time, sites, population, and resources.

Test the Recruitment Pathway Against What the Main Study Requires

In Brief

Recruitment is feasible when the available participant pool and planned recruitment process can realistically produce the required sample, including any important subgroup representation, within the main study's available time, sites, and resources.

Do not judge feasibility only by the number recruited in a pilot. Track the pathway from potential participants through screening, eligibility, contact, consent, and enrollment, including the time required and reasons people are lost at each stage. Then compare that evidence with what the full study will actually require.

03 · What You Need to Know

Recruitment Feasibility Is a Pipeline, Not a Single Percentage

Begin With the Recruitment Requirement of the Main Study

You cannot decide whether recruitment is feasible without first defining what successful recruitment would mean for the proposed study.

Suppose the main study requires 300 participants within ten months. That translates into an average of 30 participants per month if recruitment is approximately steady. If recruitment will occur at five sites, you also need to consider how many participants each site can realistically contribute and whether recruitment is expected to begin simultaneously.

The target may include more than total sample size. A study may require sufficient participants in particular age groups, settings, study arms, clusters, or other analytically important categories. Recruiting the total number while systematically missing an essential segment of the target population may not satisfy the design.

Recruitment feasibility therefore begins with explicit goals: who needs to be recruited, how many, from where, and by when.

Do Not Confuse the Target Population With the Accessible Population

A population can appear enormous on paper while the number of people you can actually recruit is much smaller.

Consider a study of university students. An institution may have 20,000 students, but perhaps only students in particular programs or year levels satisfy the eligibility criteria. Some cannot be contacted through the available channels. Others may be unavailable during the recruitment period. Still others may decline participation.

The same issue occurs in clinical, community, organizational, and school-based research. The theoretical target population is not the same as the pool that can realistically enter the study.

Before assuming recruitment is feasible, estimate the accessible population and identify the restrictions that reduce it.

Map Every Step From Identification to Enrollment

A recruitment rate calculated only from the final number enrolled can hide where the process is failing.

A more informative approach tracks the pathway. Depending on the study, this might include potential participants identified, screened, eligible, approached or contacted, responding, consenting, and finally enrolled.

Eligible Those who satisfy the study's inclusion and exclusion criteria.
Contacted or approached Eligible individuals who can actually receive the invitation.
Consented Individuals who agree to participate after receiving the required information.
Enrolled Participants who complete the requirements for entry into the study.

The exact stages depend on the design. The purpose of mapping them is to identify the denominator behind every percentage and determine where losses occur.

Calculate Rates That Answer Different Recruitment Questions

There is no single recruitment percentage that tells the whole story. Different denominators answer different questions.

Example Recruitment Measures
Eligibility rate = Eligible individuals ÷ Individuals screened
This estimates how much of the screened pool satisfies the study's eligibility criteria.
If 120 of 200 screened individuals are eligible, the observed eligibility rate is 120 ÷ 200 = 60%.
Example Recruitment Measures
Consent rate = Individuals consenting ÷ Eligible individuals approached
This indicates how frequently approached eligible individuals agree to participate, provided the numerator and denominator are defined consistently.
If 72 of 120 eligible individuals approached consent, the observed consent rate is 72 ÷ 120 = 60%.
Example Recruitment Measures
Recruitment rate over time = Participants enrolled ÷ Recruitment time
The time unit might be weeks or months and can be calculated overall or by site where appropriate.
If 72 participants are enrolled over three months, the observed average recruitment rate is 24 participants per month.

These calculations answer different questions. A low eligibility rate suggests that the initial pool contains many people who cannot enter the study. A low consent rate suggests a problem after eligible participants are approached. A slow recruitment rate despite acceptable conversion percentages may indicate that too few potential participants enter the pathway in the first place.

Time Is Part of Recruitment Feasibility

“We successfully recruited 50 participants” is incomplete without knowing how long recruitment took.

If a pilot recruits 50 participants in one month and the main study requires 300 participants over twelve months under comparable conditions, the result may be encouraging. If recruiting those same 50 participants took ten months, the implications are very different.

Track the beginning and end of recruitment periods, enrollment over time, site activation where relevant, and interruptions such as academic calendars, holidays, clinic schedules, or organizational cycles that may affect availability.

Recruitment may also be nonlinear. Initial enthusiasm, intensive researcher involvement, or a readily accessible first pool can make early recruitment faster than later recruitment. Extrapolation should therefore be cautious.

Track Why Potential Participants Are Lost

A feasibility assessment becomes much more useful when it explains why recruitment falls short.

Potential participants may be lost because they do not meet eligibility criteria, cannot be contacted, decline participation, cannot accommodate the schedule, find procedures burdensome, lack required technology, or encounter other barriers.

These reasons suggest different solutions. If most losses occur because eligibility criteria exclude otherwise suitable participants, researchers may reconsider whether every criterion is scientifically necessary. If eligible people cannot be contacted, changing the outreach method may be more relevant. If participants decline because of burden, the study procedures themselves may need attention.

Systematically tracking losses converts “recruitment was difficult” into information that can improve the main study.

Recruitment Quality Can Matter as Much as Recruitment Quantity

Meeting the overall enrollment target does not necessarily mean the recruitment strategy is adequate.

A recruitment method may disproportionately reach one part of the intended population while failing to engage others. In studies where particular groups, settings, or clusters are necessary to answer the research question, sample composition is part of feasibility.

The relevant benchmark should follow the design. Researchers should not impose demographic quotas without methodological justification, but neither should they declare recruitment successful when the resulting sample cannot support the comparisons or representation the study explicitly requires.

Test the Recruitment Strategy, Not Just Participant Willingness

Recruitment feasibility depends partly on the strategy used to reach potential participants. Email, telephone calls, clinician referral, social media, community outreach, institutional announcements, in-person recruitment, and other approaches can perform differently across populations and settings.

Preliminary testing can compare how recruitment channels contribute to identification, response, eligibility, consent, and enrollment. It can also reveal practical costs, staffing requirements, and delays.

If the pilot uses unusually intensive researcher involvement that cannot be sustained during the main study, however, its apparent recruitment success may not be scalable. The conditions used to establish feasibility should resemble the conditions available later.

Use Progression Criteria Where They Improve the Decision

For pilot and feasibility studies, researchers may specify recruitment progression criteria before data collection. These provide a basis for interpreting whether observed recruitment supports progression, requires modification, or indicates serious difficulty.

A traffic-light approach is sometimes used. A recruitment result in a green range may support proceeding, an amber range may indicate that modifications are needed, and a red range may indicate that recruitment is unlikely to support the planned study without substantial change.

The thresholds should be justified for the actual project. There is no universal recruitment percentage that makes every study feasible. Sample requirements, recruitment duration, available sites, population size, resources, and possible corrective strategies all matter.

Watch Out

Do not create an arbitrary recruitment threshold simply because progression criteria sound rigorous. Criteria should correspond to the recruitment performance the future study actually needs and should be interpreted alongside uncertainty, contextual evidence, and the feasibility of credible modifications.

Ask Whether the Pilot Conditions Can Scale

A pilot may recruit successfully because the principal investigator personally contacts every participant, one exceptionally productive site provides most of the sample, or additional staff temporarily support recruitment.

If those conditions cannot exist in the main study, the pilot has tested the wrong recruitment system.

Before extrapolating, compare the pilot and main study in terms of recruitment personnel, sites, population access, recruitment duration, eligibility criteria, incentives where applicable, communication channels, and participant burden.

The closer the conditions are to those intended for the main study, the more informative the feasibility evidence is likely to be.

Recruitment Feasibility Is Not the Same as the Decision to Proceed

A study can demonstrate that recruitment is feasible and still not proceed. Funding may disappear, the intervention may change, another feasibility problem may emerge, or the research question may become less important.

The reverse can also occur. Recruitment may fall below a prespecified target, but investigators may identify a credible modification, such as adding sites or changing a recruitment process, that makes progression reasonable.

Feasibility asks whether recruitment can work. Progression asks what researchers will do given that evidence and the wider context. The two judgments are related but not identical.

How a Recruitment Funnel Reveals the Real Problem

Hypothetical Example

Testing Recruitment for a University-Based Longitudinal Study

A research team plans a main study requiring 360 participants recruited over twelve months. Before committing to the full study, the researchers test their planned recruitment pathway for two months.

Potential participants identified 500 students enter the initial recruitment pool.
Screened 300 students complete eligibility screening.
Eligible 180 satisfy the study criteria.
Consented and enrolled 60 eligible students enroll during the two-month period.
Initial interpretation The study recruited an average of 30 participants per month. If that rate were sustained for twelve months, it would correspond to the required 360 participants.
Closer inspection Most losses occurred before screening because many identified students never responded. Recruitment also slowed substantially during the second month as examinations approached.
Decision Rather than declaring recruitment simply “feasible,” the team investigates additional contact methods and adjusts the recruitment calendar before relying on the observed average rate for the main study.

The final enrollment number is useful, but the recruitment funnel explains whether that number is likely to be reproducible and where the process may need modification.

05 · What Researchers Often Get Wrong

Common Mistakes When Judging Recruitment Feasibility

Misconception

A Large Target Population Means Recruitment Will Be Easy

The target population may be much larger than the accessible and eligible population. Eligibility restrictions, contact limitations, willingness to participate, timing, and competing demands can dramatically reduce the number who ultimately enroll.

Misconception

Reaching the Pilot Recruitment Target Proves Recruitment Is Feasible

A small target may be reached despite an unsustainable recruitment rate, intensive researcher effort, unusually favorable timing, or one highly productive site. Judge feasibility against the rate and conditions required by the main study.

Misconception

The Recruitment Rate Is Simply Recruited Divided by Invited

That may be a useful measure if both terms are clearly defined, but it is not the only recruitment indicator. Eligibility, approach, response, consent, enrollment over time, site contribution, and losses at different stages answer different feasibility questions.

Misconception

If Recruitment Is Slow, You Just Need to Invite More People

That works only when enough additional eligible and accessible people exist and the bottleneck is at the top of the recruitment funnel. If eligible participants decline because of study burden or cannot be contacted through the available system, simply expanding invitations may not solve the underlying problem.

Misconception

Recruitment Feasibility Is Only About the Total Sample Size

Some designs require particular sites, clusters, subgroups, or participant characteristics. A recruitment strategy can meet the overall number while failing to produce the sample structure required to answer the research question.

A Recruitment Threshold Automatically Determines Whether the Study Should Continue

Progression criteria can improve decision-making, but they should usually inform rather than replace judgment. Consider why the target was missed, uncertainty around the observed rate, whether credible modifications exist, and other feasibility evidence before deciding what happens next.

Build a Recruitment Funnel Before You Build a Recruitment Assumption

Start with the sample and timeline required by the main study. Work backward through the recruitment process to estimate how many eligible people must be approached, screened, or identified to produce that final number.

A simple decision framework

If you do not know how many potentially eligible participants are accessible
Estimate or test the available recruitment pool before assuming that the sample-size target is attainable.
If many potential participants disappear before eligibility can be assessed
Examine identification, contact, and response procedures rather than changing eligibility criteria automatically.
If many screened individuals are ineligible
Reassess the size of the accessible eligible population and whether every eligibility criterion is scientifically necessary.
If eligible participants frequently decline
Investigate acceptability, burden, communication, scheduling, and other reasons for non-consent where appropriate.
If the observed enrollment rate is adequate only under unusually intensive pilot conditions
Test or model a recruitment approach that can actually be sustained during the main study.
If recruitment is below target but the bottleneck has a credible remedy
Estimate whether the proposed modification is sufficient and consider testing it before committing to full-scale recruitment.

Recruitment is only one component of overall study feasibility. Even a strong recruitment pathway cannot compensate for unacceptable retention, impractical procedures, or unusable data. It should therefore be considered alongside the other study processes that may need testing before full implementation.

Before Calling Your Recruitment Plan Feasible, Check

Before committing to full recruitment, check:
How many participants does the main study require, from which populations or sites, and within what recruitment period?
How large is the genuinely accessible pool rather than the theoretical target population?
How many potential participants are identified, screened, eligible, approached, consenting, and ultimately enrolled?
What are the relevant eligibility, consent, enrollment, and recruitment-over-time rates, with clearly defined denominators?
Where are potential participants lost from the recruitment pathway, and why?
Does recruitment produce any sites, clusters, or participant groups required by the design, not merely the overall number?
Can the observed recruitment performance be sustained with the staffing, sites, channels, time, and resources available for the main study?
Have important recruitment progression criteria been justified in relation to what the main study actually requires?
If recruitment falls short, can the identified bottleneck realistically be corrected before the main study?

How do you calculate a recruitment rate?

There are several useful recruitment measures, so define the numerator and denominator explicitly. For example, you might calculate the proportion of eligible approached individuals who enroll or the number of participants enrolled per month. Different measures answer different questions about the recruitment pathway.

What is a good recruitment rate for a research study?

There is no universal percentage that defines good recruitment. Adequacy depends on how many participants the study needs, the available population, recruitment period, number of sites, resources, study burden, and design. Judge the observed rate against what your particular main study requires.

How long should recruitment be tested?

Long enough to provide useful evidence about the recruitment process and relevant variation. A very short period may be misleading if recruitment is affected by seasons, academic calendars, site activation, clinic schedules, or depletion of the easiest-to-reach participants. The appropriate duration depends on the context.

Should I track people who decline participation?

Where ethically and practically appropriate, tracking the number declining and reasons for non-participation can help identify recruitment barriers. Data collection about nonparticipants must still comply with relevant ethics, privacy, consent, and institutional requirements.

Can I extrapolate the pilot recruitment rate to the main study?

You can use it as evidence, but extrapolation should account for uncertainty and differences in scale, duration, sites, staffing, timing, recruitment channels, and population availability. Early recruitment may not remain constant throughout a longer main study.

What if recruitment is feasible but slower than planned?

Consider whether the timeline can reasonably be extended, additional sites or recruitment channels can be added, or another identified bottleneck can be addressed without compromising the scientific design. If changes are substantial, further preliminary testing may be warranted before relying on the revised plan.

Should recruitment feasibility be tested separately from a pilot study?

Not necessarily. Recruitment can be one objective within a broader pilot or feasibility study. Separate work may be useful when recruitment is the dominant uncertainty or needs to be investigated before it makes sense to test the rest of the study workflow.

Decide whether recruitment is feasible by testing the complete pathway from the accessible population to enrollment and comparing the resulting numbers, rates, timing, sample composition, and resource requirements with what the main study will need.

Do not stop at “we recruited enough people for the pilot.” Track where potential participants are lost, identify why, and ask whether the observed recruitment conditions can scale. The most useful feasibility assessment tells you not only whether recruitment is currently falling short, but where the bottleneck lies and whether a credible modification can fix it.
