Can One Researcher Conduct a Systematic Review Alone?
You may have designed the question yourself, developed the search, and be perfectly capable of reading every retrieved paper. Why, then, do systematic-review protocols so often mention two independent reviewers?
The reason is not simply workload.
Systematic reviews contain repeated judgments with direct consequences for the evidence base. A reviewer can mistakenly exclude an eligible study, misread an outcome, enter a number incorrectly, or interpret an appraisal criterion differently. Having another person independently assess important decisions creates an opportunity to detect those errors and reduces dependence on one person's judgment.
But "two reviewers" does not mean two people must independently duplicate every task from beginning to end. Requirements differ by review stage and methodology.

Why Systematic Reviews Use Independent Duplicate Procedures
The Main Purpose Is Error and Bias Reduction
Study selection is one of the most consequential stages of a systematic review. Every exclusion changes the evidence base. If an eligible study is mistakenly discarded, it may never reach extraction, appraisal, or synthesis.
Cochrane therefore requires at least two people working independently to determine whether studies meet the eligibility criteria at the final inclusion stage and requires a predefined process for resolving disagreements. Its rationale is that duplicate selection reduces mistakes and the possibility that inclusion decisions are influenced by one person's biases.
The same principle applies to extraction of outcome data. A transcription error, incorrect denominator, wrong time point, or selection of an inappropriate result can feed directly into the synthesis. Cochrane requires outcome data to be extracted independently by at least two people.
Independent Means More Than Two People Looking at the Same Decision Together
Independent assessment means that reviewers initially make their judgments without knowing or being influenced by the other reviewer's decision. Their results are then compared.
If both reviewers disagree, they discuss the study or extracted data and attempt to reach consensus. The protocol should define what happens when disagreement remains, commonly involving consultation with another reviewer or an agreed arbitration procedure.
This independence matters because two people making a decision together from the beginning may reproduce the same interpretation without revealing where their individual judgments would have differed.
Independent duplicate assessment
Two reviewers make the initial decision or extraction separately, compare their results, and then resolve discrepancies.
Verification
One reviewer completes the task and another subsequently checks the first reviewer's work. This can improve accuracy but is not methodologically identical to independent duplicate assessment.
Full-Text Eligibility Decisions Are Especially Important
Screening usually occurs in stages. Reviewers first examine titles and abstracts to remove clearly irrelevant records, then assess potentially eligible reports in full text.
Under Cochrane guidance, duplicate title-and-abstract screening is desirable but not mandatory. Initial screening can be undertaken by one person. Final determination of whether potentially eligible studies meet the eligibility criteria, however, must involve at least two people working independently.
This distinction reflects the consequences of the decision. Early screening can be designed to be deliberately inclusive, allowing uncertain records to progress. A full-text exclusion is final and directly removes a potentially eligible study from the review.
Data Extraction Is Not One Uniform Task
Researchers extract many kinds of information: study design, country, sample characteristics, intervention details, outcome definitions, numerical results, funding information, and more.
Cochrane distinguishes between study characteristics and outcome data. Independent duplicate extraction of study characteristics is highly desirable, while independent duplicate extraction of outcome data is mandatory for Cochrane intervention reviews because those data feed directly into the synthesis and conclusions.
Cochrane also notes evidence that independent extraction by two authors can result in fewer errors than extraction by one person followed by verification.
Critical Appraisal or Risk of Bias Also Benefits From Independent Assessment
Risk-of-bias assessment frequently requires methodological judgment rather than simple transcription. Two researchers can read the same study and interpret allocation procedures, missing data, selective reporting, or other design features differently.
For this reason, major evidence-synthesis methodologies commonly use two independent reviewers for methodological appraisal. JBI templates, for example, specify independent appraisal by two reviewers, with disagreements resolved through discussion or a third reviewer.
The exact appraisal process should follow the methodology and tool appropriate to the type of evidence being reviewed.
You Do Not Need Two People Independently Running the Search
Duplicate review does not mean every task should be repeated.
Cochrane specifically states that it is not necessary or desirable for two people to conduct independent searches in parallel. Instead, search strategies should ideally receive peer review from a suitably qualified and experienced librarian or information specialist before being run.
This is a useful illustration of the broader principle: methodological safeguards should target the risks associated with each task rather than duplicate work indiscriminately.

Why Not Just Let One Reviewer Work and Have Someone Check a Sample?
Partial verification can be useful in some review designs, especially when methodological guidance explicitly permits streamlined procedures. But it is not equivalent to independent duplicate assessment of every consequential decision.
If a second reviewer checks only 10% of full-text decisions, errors in the unchecked 90% can remain undetected. Sampling can estimate disagreement rates or provide quality control, but it does not protect each individual inclusion decision.
Whether such a compromise is acceptable depends on the review methodology. A rapid review, for example, may deliberately streamline certain procedures to meet a genuine time constraint. Those modifications should be planned, justified, and reported rather than quietly introduced into a conventional systematic review.
What About a Master's Thesis or PhD Conducted by One Student?
This is where methodological expectations collide with academic reality. A thesis may have one principal student researcher, but that does not necessarily mean every review task must be completed by that person alone.
A supervisor, co-supervisor, research colleague, trained research assistant, or another appropriately prepared reviewer may independently assess records or data at stages requiring duplicate review. The second reviewer needs enough training to apply the eligibility criteria or extraction rules consistently; merely signing off on the student's decisions does not reproduce independent assessment.
Cochrane itself does not publish reviews proposed to be undertaken by one person and emphasizes team-based review work partly because study selection, data extraction, and certainty assessment require multiple people.
Your university may impose different requirements from Cochrane, so institutional thesis rules and the specific review methodology both need to be checked.
Two Reviewers Do Not Guarantee Agreement
Disagreement is expected. In fact, disagreement can reveal ambiguous eligibility criteria, poorly defined outcomes, inconsistent extraction rules, or differences in how reviewers interpret an appraisal tool.
This is why pilot testing is valuable. Reviewers can independently screen or extract a small sample, compare decisions, discuss discrepancies, and clarify instructions before processing the full evidence base.
Cochrane recommends training and pilot testing for data extraction and requires protocols to define how disagreements will be resolved.
Do Not Turn Reviewer Agreement Into the Main Goal
High agreement can be reassuring, but the purpose of duplicate review is not to produce an impressive agreement statistic. Two reviewers can agree because the criteria are clear, but they can also agree on the same mistaken interpretation.
The more important objective is accurate, reproducible application of defensible criteria, with disagreements used as opportunities to identify ambiguity and error.
Watch Out
Do not report "two reviewers screened the studies" if one reviewer made all decisions and the second merely approved the final list. State whether assessment was independent, which stages were duplicated, how disagreements were resolved, and where verification rather than independent duplication was used.
Different Review Methodologies May Set Different Requirements
Cochrane standards are highly influential, but they are not universal rules governing every systematic review in every discipline. JBI, Campbell, specialist review organizations, journal policies, and discipline-specific methodologies may specify their own procedures.
That is why the defensible answer to "Do I need two reviewers?" is not simply "always" or "never." Identify the methodology you claim to follow and satisfy its requirements at each review stage.
