Are PubMed and MEDLINE the Same Database?
If you search health, medical, nursing, or life-sciences literature, you will probably encounter both PubMed and MEDLINE. They are sometimes spoken about as though the names were interchangeable. That is understandable because MEDLINE records are searchable through PubMed, and MEDLINE makes up a major part of what researchers find there.
But PubMed and MEDLINE are not two names for exactly the same resource. The distinction matters when you are designing a literature search, interpreting a journal's claim that it is "in PubMed," or trying to determine whether the records you retrieved have been indexed with Medical Subject Headings (MeSH).

How PubMed and MEDLINE Actually Relate to Each Other
What Is PubMed?
PubMed is a free database maintained by the National Center for Biotechnology Information (NCBI) at the U.S. National Library of Medicine (NLM). It supports the search and retrieval of biomedical and life-sciences literature.
One of the easiest ways to understand PubMed is to think of it as a broader literature resource that brings together citations originating from several NLM collections. According to NLM, PubMed facilitates searching across MEDLINE, PubMed Central (PMC), and selected content from NCBI Bookshelf.
This is why saying that an article is "in PubMed" does not necessarily tell you why it is there or whether it has been indexed for MEDLINE.
What Is MEDLINE?
MEDLINE is NLM's premier bibliographic database for journal literature in the life sciences, with a concentration on biomedicine. Its subject coverage extends beyond clinical medicine to areas relevant to health and biomedical research, including parts of the behavioral sciences, public health, health policy, biological sciences, chemistry, and bioengineering.
MEDLINE is a primary component of PubMed. NLM reports that MEDLINE contains more than 31 million references and currently includes citations from more than 5,200 journals worldwide. Its historical coverage extends back to the mid-twentieth century, with selected older records also available.
Journals do not become MEDLINE journals simply because their articles can be found somewhere in PubMed. MEDLINE has its own journal selection and evaluation process administered by NLM.
The Relationship Is Not PubMed Versus MEDLINE
The word "versus" in PubMed vs. MEDLINE can make the relationship sound more competitive than it really is. These are not rival databases in the way researchers might compare independent bibliographic platforms.
MEDLINE
A curated bibliographic database of journal citations and abstracts, with MEDLINE records indexed using MeSH and other indexing data.
PubMed
A broader free search and retrieval database that includes MEDLINE as well as citations from other NLM literature resources.
In other words, when you search PubMed, you can retrieve MEDLINE records, but your results may also contain records that are not indexed for MEDLINE.
What Does PubMed Contain Beyond MEDLINE?
PubMed receives citation information from several sources. In addition to MEDLINE, PubMed includes citations associated with PubMed Central and selected NCBI Bookshelf content. PubMed may also contain citations from MEDLINE journals that are not themselves indexed for MEDLINE.
For example, NLM explains that citations from a MEDLINE journal can appear in PubMed before the date on which that journal was selected for MEDLINE indexing. Certain articles from MEDLINE journals may also fall outside MEDLINE's subject scope and therefore remain in PubMed without MEDLINE indexing.

Why MeSH Is an Important Part of the Difference
A distinctive feature of MEDLINE is its use of Medical Subject Headings, or MeSH. MeSH is NLM's controlled vocabulary for describing biomedical concepts consistently.
Authors may use different words to describe essentially the same concept. Controlled vocabulary helps organize those variations under standardized concepts. This can make MEDLINE particularly valuable when constructing systematic and reproducible biomedical searches.
MEDLINE indexing was performed by human indexers for many decades. NLM transitioned MEDLINE to automated indexing in 2022 and currently uses the Medical Text Indexer-NeXt Generation algorithm. The important point for a researcher is not whether a person or algorithm assigned the terms, but that MEDLINE citations receive structured subject indexing that can be searched through PubMed.
You can combine MeSH terms with keywords rather than treating them as competing approaches. This is often useful because newly added PubMed citations may not yet have MEDLINE indexing, while other PubMed records will never receive it.
Is PubMed the Same as PubMed Central?
No. This is another common source of confusion.
PubMed is principally a database of citations and abstracts. PubMed Central, usually abbreviated PMC, is NLM's free full-text archive of biomedical and life-sciences journal literature. A PubMed record may link to an article in PMC when a free full-text version is available there.

Seeing an article in PubMed therefore does not mean that its full text is freely available. Likewise, the presence of a citation in PubMed should not automatically be interpreted as MEDLINE indexing.
Can You Search Only the MEDLINE Portion of PubMed?
Yes. NLM's PubMed User Guide specifies the MEDLINE subset search tag:
medline[sb]
Adding this to a PubMed query restricts retrieval to citations that have been indexed for MEDLINE.
For example, a researcher interested in hypertension and MEDLINE-indexed records could construct a PubMed search that combines the topic with the MEDLINE subset. Whether such a restriction is desirable depends on the purpose of the search. Restricting to MEDLINE may exclude relevant PubMed citations that have not received MEDLINE indexing.
Watch Out
Do not automatically restrict a literature review to MEDLINE merely because MEDLINE has curated journal coverage and MeSH indexing. A narrower database boundary can improve specificity, but it can also remove relevant records. Search coverage should follow the research question and review methodology rather than the prestige associated with a database name.
Why Does the Difference Matter When Evaluating Journals?
The distinction becomes especially consequential when a journal says it is "indexed in PubMed."
PubMed itself is not a journal indexing program to which publishers directly apply. NLM explains that publishers seeking discoverability through PubMed instead participate through the NLM literature databases that encompass PubMed, including MEDLINE, PMC, and Bookshelf, subject to the applicable selection processes.
A journal with content discoverable through PubMed therefore should not automatically be described as a MEDLINE-indexed journal. If MEDLINE status matters for your institution, funder, promotion requirement, or publication decision, verify the journal's actual status rather than relying on a publisher's wording.

How the Difference Changes a Literature Search
Hypothetical Example
Searching for evidence on a clinical intervention
Suppose a researcher is reviewing evidence about an intervention for a chronic health condition. The researcher searches PubMed and retrieves a set of potentially relevant citations.
Search PubMed The researcher searches PubMed using keywords and relevant MeSH terms.
Retrieve the broader PubMed set The results can include MEDLINE-indexed citations as well as eligible citations from other PubMed sources.
Restrict to MEDLINE if the research purpose requires it Adding medline[sb] limits the results to citations indexed for MEDLINE.
Compare what changes The restricted search may contain fewer records because PubMed's overall coverage extends beyond MEDLINE.
Choose the search boundary deliberately For a comprehensive review, the researcher considers whether excluding non-MEDLINE PubMed records is methodologically justified rather than assuming the narrower set is automatically better.
The lesson is not that one set of results is inherently superior. The important issue is whether the database coverage and search strategy are appropriate for the question. Researchers conducting a substantial literature review may also need to consider how specialized and multidisciplinary databases complement one another rather than assuming that one biomedical resource captures all relevant literature.

Should You Search PubMed or MEDLINE?
For most researchers using NLM's free interface, the practical starting point is PubMed. It gives you access to MEDLINE while also exposing relevant records beyond MEDLINE.
The more useful question is therefore often not "Should I use PubMed or MEDLINE?" but "Should my PubMed search include all relevant PubMed records, or do I have a methodological reason to restrict it to MEDLINE?"
A simple decision framework
If you want to search broadly across PubMed's biomedical and life-sciences coverage
Search PubMed without restricting the results to MEDLINE.
If your protocol specifically requires MEDLINE-indexed records
Use an appropriate MEDLINE search route or restrict PubMed using medline[sb] .
If you need controlled-vocabulary searching
Use MeSH strategically, while also considering keywords for records or concepts not adequately captured by MeSH alone.
If a journal claims to be "indexed in PubMed"
Determine whether the claim refers specifically to MEDLINE indexing, inclusion through PMC, or another route into PubMed.
If you are conducting a comprehensive literature review
Do not assume that either PubMed or MEDLINE alone provides complete coverage. Consider the disciplinary scope of the question and other appropriate databases.
For evidence syntheses in particular, database choice should be documented carefully. Different databases cover different journals, document types, subject areas, and indexing systems, which is one reason the same search can produce different results across databases .
If your topic extends beyond biomedicine, it may also be useful to examine whether a database actually covers the literature central to your field . PubMed is exceptionally important for many biomedical questions, but relevance to one field should not be mistaken for universal coverage of scholarly research.

Before finalizing your search, check:
Am I searching PubMed broadly, or do I specifically need MEDLINE-indexed records?
Have I considered both MeSH terms and relevant free-text keywords?
If I am limiting the search to MEDLINE, can I justify what may be excluded?
If a journal claims PubMed indexing, have I verified whether it is actually indexed for MEDLINE?
Have I avoided confusing PubMed with the full-text archive PubMed Central?
Does my research question extend into disciplines that PubMed or MEDLINE may not cover adequately?
Have I documented the database, interface, search strategy, limits, and search date sufficiently for the purpose of my review?
