Umbrella Review vs. Systematic Review: When Should You Review the Reviews?

A systematic review usually synthesizes primary studies, while an umbrella review synthesizes existing systematic reviews or research syntheses. Reviewing the reviews is most useful when enough relevant reviews already exist to answer a broader question.

01 · The Question

What If There Are Already Too Many Systematic Reviews?

Systematic reviews were developed partly to make large bodies of primary research more manageable. In some fields, however, researchers and decision-makers now face a second problem: there are so many systematic reviews that understanding the reviews themselves has become difficult.

That is where umbrella reviews enter the picture.

An umbrella review moves the unit of evidence up one level. Rather than primarily searching for individual trials, surveys, cohort studies, or other primary studies, it systematically identifies and synthesizes existing systematic reviews or research syntheses addressing a broader question.

This can provide a high-level view of a mature evidence base. It can also introduce methodological complications that do not arise in quite the same way when working directly with primary studies, particularly when several reviews contain the same studies.

Review the Reviews When the Existing Review Evidence Is the Right Unit of Analysis

In Brief

A systematic review usually identifies and synthesizes eligible primary studies to answer a defined question, whereas an umbrella review systematically identifies and synthesizes existing systematic reviews or other eligible research syntheses. An umbrella review is most useful when multiple relevant reviews already exist and you need a broader synthesis of what those reviews collectively show.

Umbrella reviews do not simply save you from reading primary studies. Their appropriateness depends on the review question, the availability and currency of suitable systematic reviews, their methodological quality, their degree of overlap, and whether review-level evidence can actually answer the question you care about.

03 · What You Need to Know

What Changes When the Review Becomes the Evidence?

A Systematic Review Usually Starts With Primary Studies

A conventional systematic review defines a research question and uses explicit, reproducible methods to identify eligible primary studies. The researchers then appraise and synthesize those studies using methods appropriate to the question and evidence.

For example, a systematic review examining whether an educational intervention improves student achievement might identify individual randomized trials and quasi-experimental studies. Those primary studies become the evidence base from which the review draws its conclusions.

A systematic review may or may not include a meta-analysis of compatible quantitative findings. Statistical pooling is a possible synthesis technique rather than the defining feature of the review.

An Umbrella Review Starts With Reviews

An umbrella review operates at a different level. JBI describes umbrella reviews as syntheses of existing research syntheses, while Cochrane uses the term overview of reviews for a closely related approach.

In a Cochrane Overview, the unit of searching, inclusion, and data analysis is the systematic review rather than the primary study. The overview systematically identifies multiple reviews addressing related questions and extracts and analyzes their findings across important outcomes.

You may also encounter terms such as review of reviews, overview of systematic reviews, meta-review, and systematic review of reviews. Terminology varies, so the methodology and eligibility criteria should make clear what kind of synthesis is actually being conducted.

Umbrella Reviews Are Especially Useful for Broad Decision Questions

Suppose a decision-maker wants to compare several interventions for the same broad problem. Separate systematic reviews already examine intervention A, intervention B, intervention C, and intervention D. Asking the decision-maker to locate and reconcile all of those reviews may defeat part of the purpose of evidence synthesis.

An umbrella review can bring those review-level findings together. Cochrane notes that overviews can describe the current body of systematic review evidence or address a new review question that was not itself the focus of the included systematic reviews.

This higher-level perspective can be particularly useful when the decision involves choosing among several alternatives rather than determining whether one narrowly defined intervention works.

Reviewing Reviews Does Not Automatically Mean Less Work

The phrase "review of reviews" can sound like an efficient shortcut. Sometimes it is more efficient than starting again from thousands of primary records, but it introduces another layer of complexity.

Researchers need to determine what qualifies as an eligible systematic review, assess the methodological quality or risk of bias of those reviews, extract findings consistently, reconcile different definitions and outcomes, evaluate how current each review is, and decide how to handle conflicting conclusions.

They must also understand enough about the underlying primary evidence to avoid treating each review as an entirely independent piece of information.

Overlap Is One of the Central Problems

Imagine that an umbrella review includes five systematic reviews. Review A contains 20 primary studies, Review B contains 18, and Review C contains 25. Those numbers do not necessarily represent 63 different studies. The same influential trials may appear in all three reviews.

If overlapping reviews are treated as independent bodies of evidence, the same primary study can effectively influence the umbrella review several times. This can create a misleading impression of how much independent evidence exists.

Cochrane therefore identifies management of overlapping systematic reviews as a specific methodological issue in overviews. Authors may need to examine the primary-study composition of included reviews and use an explicit approach to dealing with overlap.

Watch Out

Five systematic reviews reaching similar conclusions do not necessarily represent five independent bodies of evidence. They may substantially overlap in the primary studies they include. Always investigate review overlap before interpreting the number of reviews as evidence of replication.

The Umbrella Review Inherits Limitations From the Reviews Below It

An umbrella review cannot automatically repair methodological weaknesses in its included systematic reviews. If the underlying reviews searched poorly, used inappropriate eligibility criteria, failed to assess risk of bias adequately, or are several years out of date, those limitations constrain what the umbrella review can conclude.

This is why critical appraisal occurs at the review level. JBI provides a critical appraisal checklist for systematic reviews and research syntheses as part of its umbrella-review methodology.

The umbrella review should also consider the quality or risk-of-bias information relating to the primary studies where relevant and available. In practice, this can be difficult because different reviews may use different appraisal tools, thresholds, and reporting conventions.

Currency Matters More Than the Publication Date Alone

A systematic review published recently may have conducted its final search considerably earlier. Conversely, an older review may still cover a field in which little new evidence has appeared.

When deciding whether review-level evidence is sufficient, examine the search dates and scope of the existing reviews, not merely the year printed on the article. If the reviews do not include important recent primary evidence, an umbrella review based solely on them may provide an outdated picture.

This issue is closely related to deciding whether an existing review is recent enough to affect the justification for another review.

An Umbrella Review Is Not a Meta-Analysis of Meta-Analyses

An umbrella review can report or analyze quantitative results from included reviews, but its defining feature is the systematic synthesis of review-level evidence, not a requirement to combine pooled estimates into another pooled estimate.

Statistically combining meta-analytic results can be methodologically difficult because included reviews may overlap in their primary studies, use different eligibility criteria, analyze different effect measures, or employ different statistical models. Treating their pooled estimates as independent can be inappropriate.

As with primary-study systematic reviews, the synthesis method should follow the question and structure of the evidence rather than the desire to produce one impressive-looking number.

An Umbrella Review Needs Its Own Protocol and Methods

Using published reviews as evidence does not remove the need for prospective methodological planning. JBI umbrella-review guidance covers development of an umbrella-review protocol, inclusion criteria, search strategy, critical appraisal, data extraction, synthesis, and reporting.

For overviews of reviews of healthcare interventions, the PRIOR statement provides dedicated reporting guidance. PRIOR includes 27 main items with additional sub-items intended to improve complete and transparent reporting of overviews.

Reporting guidance should be matched to the type and field of the overview. PRIOR was specifically developed for overviews of reviews of healthcare interventions, so it should not automatically be treated as a universal reporting standard for every form of umbrella review.

When Reviewing the Reviews Makes More Sense Than Starting Again

Hypothetical Example

Comparing Approaches to Improving University Student Well-Being

A research team wants to understand the evidence for several categories of interventions intended to improve university students' psychological well-being.

Existing evidence The team finds multiple systematic reviews covering psychological interventions, physical-activity interventions, digital interventions, mindfulness-based approaches, and institutional programs.
Review question Rather than asking whether one narrowly defined intervention works, the team wants a higher-level synthesis of what the existing systematic-review evidence suggests across intervention categories.
Umbrella approach The researchers systematically identify eligible reviews, appraise their methodological quality, extract relevant findings, examine overlap among their included primary studies, and compare conclusions across intervention categories.
Result The final synthesis shows where review-level evidence is relatively consistent, where findings conflict, which intervention categories lack adequate synthesis, and where existing reviews are outdated or methodologically weak.

If instead the researchers wanted to answer a narrowly defined question for which no adequate systematic review existed, going directly to the primary studies would probably make more sense.

05 · What Researchers Often Get Wrong

Common Misunderstandings About Umbrella Reviews

Misconception

An Umbrella Review Is Simply a Very Large Systematic Review

Size is not the defining distinction. The important difference is the unit of inclusion and synthesis. A systematic review usually works directly with primary studies, whereas an umbrella review works primarily with existing systematic reviews or other eligible research syntheses.

Misconception

If Several Systematic Reviews Agree, the Evidence Must Be Strong

Agreement among reviews can be informative, but the reviews may share many of the same primary studies and methodological limitations. Confidence should depend on the underlying evidence, review quality, overlap, consistency, and other relevant considerations rather than simply counting how many reviews reached the same conclusion.

Misconception

An Umbrella Review Saves You From Looking at Primary Studies

The systematic reviews are the principal units of inclusion, but understanding their primary-study composition can be necessary for assessing overlap, currency, eligibility differences, and interpretation. Review-level synthesis does not mean the underlying studies become irrelevant.

Misconception

You Should Conduct an Umbrella Review Whenever Two Systematic Reviews Exist

The existence of multiple reviews is not enough. They must be sufficiently relevant to the umbrella question, and the resulting synthesis should add useful understanding rather than merely repeat their conclusions. Review quality, scope, currency, and overlap also matter.

Misconception

An Umbrella Review Is Automatically the Highest Level of Evidence

Evidence hierarchies can be useful shorthand, but the label alone does not establish trustworthiness. An umbrella review built from outdated or poorly conducted systematic reviews can provide a weaker basis for inference than a rigorous, current systematic review of appropriate primary studies.

Use an Umbrella Review When the Review-Level Evidence Can Answer Your Question

Before choosing an umbrella review, search for existing systematic reviews and inspect them closely. The question is not merely whether reviews exist but whether they collectively form an appropriate evidence base for the synthesis you intend to conduct.

A simple decision framework

If multiple relevant systematic reviews already cover related parts of a broader decision question
An umbrella review may provide a useful higher-level synthesis.
If your question requires evidence from individual primary studies and existing reviews do not answer it
Conducting a systematic review of primary studies is likely to be more appropriate.
If existing reviews are substantially outdated
Consider whether an updated systematic review of primary evidence would provide a more current answer.
If existing reviews are methodologically weak
Do not assume that placing them inside an umbrella review will correct their weaknesses.
If several eligible reviews contain many of the same primary studies
Plan explicitly how overlap will be identified, quantified where useful, managed, and reflected in interpretation.

Before committing to the design, also consider the broader family of review methodologies available for different evidence-synthesis questions. An umbrella review is useful because of what it can answer, not because it occupies another layer above a systematic review.

Before Starting an Umbrella Review

Before reviewing the reviews, check:
Define a question for which existing systematic reviews or research syntheses are an appropriate unit of evidence.
Search sufficiently to establish that multiple relevant reviews actually exist.
Inspect the search dates of candidate reviews to determine whether the evidence is sufficiently current.
Develop explicit criteria defining what qualifies as an eligible systematic review or research synthesis.
Plan an appropriate method for critically appraising the included reviews.
Determine how overlapping primary studies across reviews will be identified and handled.
Develop the protocol before study selection and synthesis decisions are made.
Use methodological and reporting guidance appropriate to the type of umbrella review and disciplinary context.

Questions About Umbrella and Systematic Reviews

Is an umbrella review the same as an overview of reviews?

The terms are often used for closely related forms of evidence synthesis, although terminology and eligibility conventions vary among methodological traditions. Cochrane uses "overview of reviews," while JBI provides specific guidance for "umbrella reviews." Authors should state the methodology they followed rather than assume every label is universally interchangeable.

Does an umbrella review include primary studies?

The principal units of inclusion in an umbrella review are existing systematic reviews or other research syntheses as defined by the methodology. Some overview methodologies may permit supplementary primary studies under specified circumstances, but this should not be assumed. Follow the eligibility rules of the methodology you are using and report any departures clearly.

How many systematic reviews do I need for an umbrella review?

There is no universally meaningful minimum number that automatically justifies the design. The important issue is whether enough relevant and appropriate review-level evidence exists to answer the umbrella question and whether synthesizing it adds value.

Can an umbrella review include meta-analyses?

Yes. Many eligible systematic reviews may contain meta-analyses, and an umbrella review can extract and compare their quantitative findings. Combining pooled estimates further requires careful methodological justification, particularly when the underlying primary studies overlap.

What if two included systematic reviews contain the same studies?

This is a common methodological problem rather than an unusual exception. Authors should identify and manage overlap explicitly because treating overlapping reviews as independent can effectively give the same primary studies repeated influence in the synthesis.

Can I conduct an umbrella review if the existing systematic reviews are old?

Possibly, but examine their last search dates and whether important primary evidence has appeared since then. If the available reviews no longer represent the current evidence base, a new or updated systematic review may be more informative.

Does an umbrella review need a protocol?

Yes, prospective methodological planning is important. JBI umbrella-review methodology explicitly includes protocol development, and the protocol should establish the question, inclusion criteria, search, appraisal, extraction, and synthesis methods before the review proceeds.

Should I use PRISMA or PRIOR for an umbrella review?

Use reporting guidance appropriate to your review and field. PRIOR was specifically developed for overviews of reviews of healthcare interventions. PRISMA 2020 primarily addresses systematic reviews of studies. Consult the methodological framework and target journal rather than assuming that one reporting checklist applies universally to every umbrella review.

A systematic review usually synthesizes primary studies, while an umbrella review synthesizes existing systematic reviews or other eligible research syntheses. Choose an umbrella review when multiple suitable reviews already exist and collectively provide the right level of evidence for the broader question you need to answer.

Reviewing the reviews does not remove methodological difficulty. You still need to assess review quality, currency, conflicting conclusions, and especially overlap among primary studies. When the available reviews are outdated, poor quality, or unable to answer your question, returning to the primary evidence may be the more defensible approach.
