What Exactly Should a Feasibility Study Tell You?

A feasibility study should reduce specific uncertainties about whether and how a proposed main study can be conducted. Its findings should help researchers decide whether to proceed, modify the design, undertake further feasibility work, or reconsider the study.

01 · The Question

What Information Should You Actually Get From a Feasibility Study?

“We will conduct a feasibility study first” sounds methodologically sensible, but it leaves the most important question unanswered: feasible in what respect?

A future study can fail for many reasons. The target population may be too difficult to recruit. Participants may find the procedures unacceptable. An intervention may be difficult to deliver consistently. Follow-up may produce too much missing data. Study sites may lack the required staff, equipment, or time. A protocol that appears workable on paper may become impractical when implemented.

A useful feasibility study identifies these uncertainties before the main study and collects evidence that can inform what happens next. Its purpose is not merely to demonstrate that some preliminary research was conducted. It should tell you something consequential about whether and how the proposed research can proceed.

A feasibility study should tell you whether important aspects of a proposed future study are workable, what problems remain, what modifications may be needed, and whether the available evidence supports proceeding to the next stage of research.

Exactly what it should measure depends on the uncertainties in your proposed study. Recruitment, retention, acceptability, intervention delivery, resources, data collection, and study procedures may matter, but they should be investigated because they represent genuine uncertainties, not because every feasibility study must assess the same checklist of outcomes.

03 · What You Need to Know

Turn Uncertainty Into Answerable Feasibility Questions

Begin With the Uncertainties in the Future Study

A feasibility study should be designed backward from the decision that researchers will eventually need to make. Before choosing outcomes, identify what is genuinely uncertain about the proposed main study.

Suppose you intend to recruit participants from several universities. The existence of a large student population does not establish that recruitment is feasible. You may need to know how many students satisfy the eligibility criteria, how many can actually be approached, what proportion consent, and how quickly recruitment occurs.

If the main uncertainty concerns a complex intervention, the relevant questions may be different. Can it be delivered as planned? Do participants engage with it? Can staff implement it consistently? Does it fit within the intended setting?

There is therefore no universal set of feasibility outcomes that every study must collect. The study should investigate the uncertainties that could materially affect the future research.

Can You Recruit the Participants You Need?

Recruitment is one of the most common feasibility concerns because an otherwise excellent study cannot answer its research question if it cannot enroll an adequate sample.

Useful recruitment information may include the number of potentially eligible participants, the proportion who satisfy eligibility criteria, the proportion approached, the proportion consenting, recruitment per site or per unit of time, and reasons people decline when those data can appropriately be collected.

The most useful measure depends on the bottleneck. A low final recruitment rate could reflect a small eligible population, poor access to that population, low willingness to participate, restrictive eligibility criteria, or an ineffective recruitment strategy. Knowing only the number recruited may therefore conceal the actual problem.

When recruitment is a major uncertainty, investigate whether the proposed recruitment strategy can realistically produce the required sample before committing to the main study.

Will Participants Stay in the Study and Complete What Is Required?

Recruitment gets participants into a study. Retention and adherence determine whether many of them remain long enough to provide the information the design requires.

A feasibility study may therefore examine withdrawal, follow-up completion, missed assessments, adherence to intervention or study procedures, and the amount and pattern of missing data. These measures become particularly important in longitudinal research, repeated-measures designs, and studies that impose substantial participant burden.

A high recruitment rate can coexist with poor retention. If participants readily enroll but routinely fail to complete later assessments, increasing recruitment alone may not solve the problem.

Are the Study Procedures Acceptable to Participants and Other Stakeholders?

A procedure can be technically possible yet unacceptable in practice. Participants may regard an assessment as too intrusive, burdensome, confusing, or time-consuming. Staff may consider an intervention incompatible with ordinary workflow. Institutions may be unwilling to support particular procedures.

Acceptability can sometimes be assessed quantitatively, but qualitative evidence may explain why a procedure succeeds or fails. Interviews, observations, open-ended responses, or process evaluation can reveal problems that a completion percentage alone cannot explain.

For participant-facing procedures, it may also be necessary to establish whether participants understand what the study asks them to do. Apparent non-adherence may sometimes reflect unclear instructions rather than unwillingness to participate.

Can the Intervention or Procedure Be Delivered as Intended?

When a future study evaluates an intervention, feasibility may depend on whether the intervention can actually be implemented according to the protocol. Relevant questions might concern delivery, fidelity, dose, attendance, adherence, staff training, equipment, scheduling, or variation across sites.

This distinction matters because an intervention can be acceptable in principle while proving difficult to deliver consistently. Conversely, researchers may implement it exactly as planned while participants engage with it poorly.

A feasibility study should therefore distinguish problems with the intervention itself from problems with the research processes used to evaluate it.

Can You Collect the Data the Main Study Will Require?

A protocol may specify what variables will be collected without establishing that collecting them is practical.

Researchers may need to examine whether assessments can be completed within the intended time, whether instruments produce excessive missing data, whether measurements can be performed consistently, whether data from different systems can be linked correctly, or whether scheduled follow-up occurs at the intended intervals.

The question is not merely whether data can be collected at all. It is whether the planned procedures can produce sufficiently complete and usable data under realistic study conditions. Where this remains uncertain, examining whether the proposed data-collection procedures are practical can prevent serious problems later.

Are the Required Resources and Study Processes Realistic?

Feasibility can also depend on resources that are easy to underestimate during protocol development. These may include staff time, training requirements, equipment availability, space, site capacity, administrative processes, scheduling, data management, and the time required to complete individual procedures.

A process that works when a senior researcher personally supervises every case may not scale to a larger study. Likewise, a procedure that takes 20 minutes longer than anticipated may appear trivial during preliminary testing but become a major staffing problem when repeated hundreds of times.

The feasibility question should therefore consider not only whether something can be done, but whether it can be done under conditions sufficiently similar to those planned for the main study.

Feasibility Findings Should Lead to a Progression Decision

Collecting feasibility outcomes is only half of the task. Researchers also need to decide what those outcomes mean for the future study.

For some studies, this can be formalized through progression criteria specified before the feasibility data are collected. Criteria may define circumstances supporting progression, progression with modifications, additional feasibility work, or a decision not to proceed with the proposed design.

Feasibility outcome The evidence collected about an uncertainty, such as recruitment rate, follow-up completion, intervention fidelity, or acceptability.
Progression criterion A prespecified basis for interpreting feasibility evidence when deciding whether and how the research should move forward.

These criteria should be justified for the particular study. A recruitment rate acceptable for one design may be inadequate for another because sample requirements, recruitment periods, available sites, and resources differ.

Progression decisions also need not be reduced mechanically to one number. Quantitative results, uncertainty around estimates, qualitative evidence, safety considerations, and the feasibility of proposed modifications may all inform the final judgment.

A Feasibility Study Can Legitimately Tell You Not to Proceed

Researchers naturally hope that preliminary work will support the main study. That expectation can create a subtle bias toward interpreting almost any result as “feasible with minor modifications.”

But a well-designed feasibility study should be capable of producing an unfavorable answer. It may show that recruitment is insufficient, participant burden is unacceptable, the intervention cannot be delivered reliably, or required resources exceed what is available.

That does not make the feasibility study unsuccessful. It may instead prevent a larger study from consuming substantially more resources while remaining unable to answer its research question.

Watch Out

Do not define feasibility retrospectively so that whatever happened appears acceptable. Identify the important uncertainties and, where appropriate, progression criteria before seeing the results. Otherwise, the decision to proceed can become a post hoc justification rather than an evidence-informed judgment.

Hypothetical Example

Preparing a Longitudinal Student Well-Being Study

A research team plans a large study in which university students complete a baseline assessment and three follow-ups across an academic year. The researchers are concerned that recruitment and repeated participation may be difficult.

Uncertainty The team does not know whether enough eligible students can be recruited within the available period or whether participants will continue completing later assessments.
Feasibility questions How quickly can eligible students be recruited? What proportion of enrolled participants complete each follow-up? Why do participants miss later assessments?
Measures The researchers track eligibility, invitations, consent, recruitment over time, follow-up completion, missing assessments, and participant feedback about study burden.
Interpretation Recruitment proves workable, but follow-up completion declines substantially because later assessments coincide with examination periods.
Decision Rather than concluding simply that the study is “feasible,” the researchers revise assessment timing and participant-contact procedures before deciding whether to proceed with the larger study.

The feasibility study has done more than generate percentages. It has identified which part of the design works, which part does not yet work adequately, why the problem may be occurring, and what needs to change.

Common Mistakes When Interpreting Feasibility

Misconception

Feasibility Means Asking Whether the Entire Study Is Possible

That question is usually too broad to be useful. Break feasibility into the uncertainties that matter for the proposed study, such as recruitment, retention, acceptability, intervention delivery, data collection, resources, or specific procedures.

Misconception

Recruiting the Target Number Means the Study Is Feasible

Recruitment is only one dimension. A study may recruit successfully while experiencing unacceptable attrition, poor adherence, incomplete measurements, inconsistent intervention delivery, or impractical resource demands.

Misconception

A Feasibility Study Should Show Whether the Intervention Works

The primary purpose of feasibility research conducted before a definitive effectiveness study is to resolve uncertainties about the future study, not to provide a definitive test of effectiveness. Small preliminary samples may provide highly imprecise effect estimates.

Every Feasibility Study Should Measure the Same Outcomes

Feasibility outcomes should correspond to the uncertainties in the proposed research. Recruitment may be critical in one project and already well established in another. Collecting standard measures without asking whether they inform a real decision can produce data without useful evidence.

Design the Feasibility Study Around the Decision You Need to Make

Write down the uncertainties before deciding what data to collect. For each one, ask what evidence would make you more or less confident about proceeding with the proposed main study.

A simple decision framework

If you are uncertain whether enough eligible participants can be enrolled
Measure the recruitment pathway and rate, not merely the final number recruited.
If participant burden or acceptability is uncertain
Examine completion and adherence alongside evidence explaining participants' experiences and difficulties.
If implementation is uncertain
Assess whether the intervention or procedure can be delivered with the required consistency, resources, and fidelity.
If data collection is uncertain
Examine completion, missingness, timing, measurement procedures, and whether the resulting data are usable for the intended analysis.
If several feasibility conditions determine whether the main study can proceed
Specify how those findings will be considered together when making the progression decision.

Keep the feasibility objectives distinct from the substantive objectives of the eventual main study. If you need to reproduce part or all of the future study to answer these questions, you may be moving into pilot study territory within the broader assessment of feasibility.

Most importantly, make the study capable of changing your plans. A feasibility study that could produce only one conclusion, “proceed as intended,” is not genuinely testing feasibility.

Before collecting feasibility data, check:
Have you identified the specific uncertainties that could affect whether or how the future study proceeds?
Does each feasibility objective correspond to one of those uncertainties?
Have you selected measures capable of answering each feasibility question?
Where percentages or rates matter, have you defined their numerator, denominator, and relevant time period clearly?
Would qualitative evidence help explain why a procedure is acceptable, impractical, or difficult to implement?
Have you considered recruitment, retention, data completeness, implementation, and resources only where they represent genuine uncertainties?
Have you specified and justified progression criteria where they would improve the decision process?
Can the findings realistically lead to proceeding, modifying the study, doing further feasibility work, or deciding not to proceed?

What is the main outcome of a feasibility study?

There is no single outcome appropriate to every feasibility study. The outcomes should address the important uncertainties about the future study. Depending on the design, these could concern recruitment, retention, acceptability, implementation, adherence, data collection, resources, or other study processes.

Does a feasibility study need progression criteria?

Progression criteria can be particularly useful when the findings will inform a decision about moving to a definitive study. They provide a prespecified basis for deciding whether to proceed, modify the design, undertake further work, or reconsider progression.
