Narrative vs. Systematic Review: When Is a Systematic Approach Actually Necessary?

A systematic review is not automatically the best choice for every literature question. The appropriate approach depends on what you need the review to establish, how comprehensively the evidence must be identified, and how much methodological transparency the conclusion requires.

01 · The Question

Does a Rigorous Literature Review Have to Be Systematic?

You have a topic, a growing collection of papers, and a decision to make: should you write a narrative review, or do you need the considerably more structured process of a systematic review?

The temptation is to treat the choice as a hierarchy. Narrative review sounds informal; systematic review sounds rigorous. From that perspective, choosing a systematic review can seem like the academically safer option.

That is the wrong decision rule.

Narrative and systematic reviews are suited to different purposes. A systematic approach becomes particularly important when your conclusion depends on identifying and evaluating an evidence base as comprehensively and transparently as possible. A narrative approach may be more suitable when the purpose is broader, conceptual, interpretive, historical, theoretical, or educational and exhaustive identification of every eligible study is not what gives the review its value.

Use a Systematic Review When the Question Requires Systematic Evidence Identification

In Brief

A systematic review is particularly appropriate when you need to answer a clearly defined question by identifying, selecting, appraising, and synthesizing the relevant evidence through explicit, prespecified, and transparent methods. A narrative review may be more appropriate when the purpose is broader interpretation, conceptual integration, theoretical discussion, historical development, or an expert overview rather than an exhaustive answer to a tightly specified evidence question.

Neither label guarantees quality. A poorly conducted systematic review can mislead, while a well-constructed narrative review can make a valuable scholarly contribution. The method should follow the question and intended claim.

03 · What You Need to Know

What Actually Separates Narrative and Systematic Reviews?

The Difference Is More Than Whether You Use a Database Search

Both narrative and systematic reviews examine existing literature. Both can involve database searching, critical reading, comparison, and synthesis. The meaningful distinction lies in how the evidence base is constructed and how strongly the review attempts to reduce the influence of reviewers' discretionary choices on what gets included and how conclusions are reached.

A systematic review works from an explicit review question and a planned methodology. Eligibility criteria, information sources, search methods, study selection, data collection, appraisal where applicable, and synthesis procedures are specified and reported systematically. This allows readers to inspect how the evidence moved from the potentially available literature to the studies that actually informed the conclusions.

A narrative review is typically more flexible. Authors may organize literature around concepts, theories, historical developments, controversies, or themes rather than attempting exhaustive identification of every study meeting prespecified criteria. This flexibility can be valuable, but it also means that readers may have less ability to determine why particular sources were included and others were not unless the authors describe their approach clearly.

Systematic Does Not Mean Statistical

A systematic review is defined by its review methodology, not by whether it performs a meta-analysis. After eligible studies have been identified, reviewers may discover that statistical pooling is inappropriate because studies differ substantially in populations, interventions, outcomes, designs, measurements, or other characteristics.

The findings can still be synthesized without meta-analysis. Conversely, statistical pooling does not by itself make a project a well-conducted systematic review. The relationship between systematic review and meta-analysis is therefore important: systematic review concerns the broader evidence-identification and synthesis process, while meta-analysis is a statistical technique that may be used within that process when appropriate.

Systematic Review Methods Help Address Selection Bias in the Review Process

Imagine that 30 studies have investigated an intervention. Ten report striking benefits, while the others report smaller, mixed, null, or unfavorable findings. If a reviewer informally selects the most visible or memorable studies, the resulting account could provide a distorted impression of the evidence.

Systematic methods attempt to reduce this problem by defining in advance what evidence is eligible and using explicit procedures to identify and select it. They cannot eliminate every source of bias. Relevant studies may remain unpublished or difficult to locate, database indexing is imperfect, reporting within primary studies can be incomplete, and reviewers still make judgments. Systematic methods make many of those decisions more visible and auditable rather than making the review automatically unbiased.

Narrative Reviews Have Different Strengths

Some questions are poorly served by reducing the task to a narrowly specified set of eligible studies. Suppose you want to explain how a theoretical concept has changed over several decades, compare competing intellectual traditions, trace the historical development of a research field, or provide an expert synthesis connecting literatures that use different terminology.

A narrative approach can permit greater interpretive flexibility. Authors can connect seminal work with recent evidence, examine conceptual disagreements, and organize a field around ideas rather than around a fixed evidence-selection pipeline.

That flexibility should not become an excuse for arbitrary citation. A credible narrative review still needs a defensible scope, thoughtful source selection, critical engagement with contrary evidence, and sufficient transparency for readers to understand the basis of its interpretation.

When Does a Systematic Approach Become Particularly Important?

The stronger your claim depends on the completeness and unbiased identification of the relevant evidence, the stronger the case for systematic methods becomes.

This is particularly important when a review is intended to establish the balance of evidence about a defined question, inform clinical or policy decisions, estimate effects, compare interventions, assess diagnostic performance, evaluate associations, or support conclusions where missing a substantial portion of eligible evidence could materially alter the answer.

A systematic approach can also be appropriate outside health research. Education, psychology, management, computer science, social science, and many other disciplines use systematic reviews when the research question and intended inference warrant structured evidence identification and synthesis.

A Broad Question Does Not Automatically Mean Narrative Review

Sometimes the question is broad because the researcher is trying to map what evidence exists rather than determine the effect of a specific intervention. In that situation, the choice may not actually be narrative versus systematic review. A scoping review may be a better methodological fit.

Scoping reviews use structured and transparent methods to examine the extent, characteristics, concepts, or types of evidence in a field. If your real objective is to map a broad evidence base, consider whether the decision is better framed as systematic review versus scoping review.

PRISMA Does Not Turn a Narrative Review Into a Systematic Review

PRISMA is a reporting guideline designed to improve transparent reporting of systematic reviews. PRISMA 2020 includes a checklist and flow diagrams that help authors report why a systematic review was conducted, what methods were used, and what was found.

It is not a certification system, and completing a PRISMA flow diagram after an informal literature search does not retroactively make the underlying review systematic. Methodological decisions need to be built into the review process itself.

Watch Out

Do not choose a systematic review merely because you believe journals will regard the word "systematic" as more rigorous. Systematic reviews impose methodological obligations. If you cannot justify the search, eligibility criteria, selection process, synthesis, and other procedures required by your chosen methodology, the label can expose weaknesses rather than conceal them.

There Is a Continuum of Structure, but Labels Still Matter

Real review practice is not always divided neatly into two boxes. Narrative reviewers may conduct structured database searches. Systematic reviews may rely on narrative synthesis rather than statistical pooling. Other methodologies occupy different positions and serve different purposes, including scoping, integrative, rapid, umbrella, and qualitative evidence syntheses.

This is why selecting among the major types of research review should begin with the question the review needs to answer rather than with a preferred label.

One Topic Can Require Two Very Different Review Designs

Hypothetical Example

Reviewing Research on Online Learning

Two researchers are interested in online learning in higher education, but they want to make different claims.

Researcher A: Conceptual question The researcher wants to explain how scholarly thinking about student engagement in online learning has evolved, compare major theoretical perspectives, and identify tensions in how engagement is conceptualized. A carefully constructed narrative review may suit this interpretive purpose.
Researcher B: Effectiveness question The researcher wants to determine whether a specified online instructional intervention improves a defined learning outcome compared with a comparator in a particular population. The answer depends on systematically identifying the eligible evidence rather than selecting illustrative studies. A systematic review is much easier to justify.

The subject matter is not what determines the review type. Both researchers are studying online learning. Their questions, intended conclusions, and evidentiary requirements are different.

Common Mistakes When Choosing Between Narrative and Systematic Reviews

Misconception

Systematic Reviews Are Always Better Than Narrative Reviews

They answer different kinds of questions. Systematic methods provide important advantages when the review needs transparent and comprehensive evidence identification, but those procedures do not automatically produce a better answer to a broad conceptual or historical question. Methodological fit matters more than placing review types on a simple hierarchy.

Misconception

Narrative Review Means You Can Cite Whatever You Want

Methodological flexibility does not remove scholarly responsibility. Selective citation can produce a distorted account of a field. Narrative reviewers should define the scope of their discussion, engage fairly with competing evidence and perspectives, and explain their approach when readers need that information to evaluate the synthesis.

Misconception

A Database Search Makes the Review Systematic

Searching Scopus, Web of Science, PubMed, or another database is only one part of a systematic review. A systematic approach also requires an explicit question, eligibility criteria, documented selection procedures, planned data collection and synthesis, and other methodological decisions appropriate to the review.

Following PRISMA Means You Conducted a Systematic Review

PRISMA helps researchers report systematic reviews transparently. It does not replace the methodology used to conduct the review. A PRISMA checklist or flow diagram cannot compensate for a review process that was not designed systematically.

Misconception

A Systematic Review Must Include a Meta-Analysis

No. Meta-analysis is a statistical synthesis technique and is appropriate only when quantitative results can reasonably be combined. A systematic review may instead use another synthesis approach. Forcing incompatible studies into a pooled estimate does not make the review more rigorous.

A Narrative Review Does Not Need Any Methodological Transparency

Expectations vary among disciplines and journals, but readers still benefit from knowing how the scope was determined and how literature was identified or selected, particularly when those choices materially influence the conclusions. Narrative does not have to mean opaque.

Choose According to the Claim You Need the Literature to Support

A useful way to choose is to work backward from the conclusion you hope the review will be capable of supporting.

A simple decision framework

If you need to determine the overall evidence relating to a clearly defined research question
A systematic review is usually the stronger methodological choice because the answer depends on explicit and defensible identification of eligible evidence.
If the review may inform consequential clinical, professional, policy, or practice decisions
Strongly consider systematic methods appropriate to the question so that evidence selection and synthesis can be scrutinized.
If you want to estimate or compare quantitative effects across studies
A systematic review is generally needed to establish the evidence base before deciding whether meta-analysis is appropriate.
If your purpose is primarily conceptual, theoretical, historical, explanatory, or interpretive
A narrative review may be appropriate, provided its scope, source selection, and claims are handled critically and transparently.
If your question is intentionally broad because you want to map the extent or characteristics of available evidence
Consider a scoping review rather than assuming the only alternatives are narrative and systematic reviews.
If you want systematic methods but cannot realistically complete the required work
Reconsider the question, scope, review methodology, resources, and timeline rather than performing a partial systematic review while retaining the label.

Also check whether a sufficiently recent and methodologically adequate review already answers your question. Before committing months of work to another systematic review, determine whether the question has already been reviewed recently enough to affect the justification for a new review.

Before Choosing Narrative or Systematic Review

Before selecting the review design, check:
Write the actual question or purpose of the review before choosing the methodology.
Determine whether your conclusion depends on identifying the eligible evidence as comprehensively as feasible.
Decide whether the review is primarily answering a focused evidence question or providing conceptual, theoretical, historical, or interpretive synthesis.
Check whether a scoping or another review methodology fits the purpose better than either of the two options.
Search for existing reviews before investing in a new systematic review.
If choosing a systematic review, develop the methods prospectively rather than reconstructing them after searching.
Confirm that you have sufficient time, expertise, database access, and reviewer capacity for the methodology you plan to use.
Use the review label that accurately describes the work you actually conducted.

Questions About Narrative and Systematic Reviews

Is a narrative review less rigorous than a systematic review?

Not in every meaningful sense. Systematic reviews provide greater methodological structure for identifying and selecting evidence, which is essential for some questions. Narrative reviews can still involve rigorous critical reasoning and synthesis. Their greater flexibility, however, can make source-selection decisions less reproducible and more susceptible to selection bias if those decisions are not handled carefully.

Can a narrative review have a search strategy?

Yes. Narrative reviewers may use structured database searches and explain how literature was identified. Doing so can improve transparency, but using a search strategy alone does not transform the work into a systematic review.

Does a systematic review have to include every paper ever published on the topic?

No. It seeks to identify evidence meeting predefined eligibility criteria relevant to its question. Studies outside those criteria are intentionally excluded, and even comprehensive searching cannot guarantee that every eligible record will be discovered. The goal is a systematic, transparent, and defensible process rather than an impossible guarantee of perfect retrieval.

Does a systematic review always require a meta-analysis?

No. Meta-analysis should be performed only when statistical pooling is appropriate. A systematic review can use other synthesis approaches when the included evidence cannot or should not be combined quantitatively.

Can I use PRISMA for a narrative review?

PRISMA 2020 was developed for reporting systematic reviews, with extensions available for particular evidence-synthesis contexts. It should not be used to imply that a narrative review was systematic when it was not. Narrative reviews should follow reporting expectations appropriate to their methodology, discipline, and target journal.

Is a systematic review necessary for a thesis or dissertation?

Not simply because the work is a thesis or dissertation. The appropriate review methodology depends on the research question, program expectations, disciplinary conventions, and intended contribution. A thesis may contain a conventional literature review, use a systematic review as one component, or make an evidence synthesis the primary study.

What if my topic is too broad for a systematic review?

First reconsider the purpose rather than merely narrowing the topic mechanically. If you want to map the breadth, characteristics, concepts, or types of evidence available, a scoping review may fit better. If your objective is conceptual or interpretive, a narrative approach may be defensible. If you genuinely need a systematic answer to a focused question, refining the question may be necessary.

Do systematic reviews require two reviewers?

Reviewer requirements depend on the methodology, stage of the review, and applicable guidance. Independent or duplicate processes are commonly used to reduce errors and individual judgment in consequential stages. The practical and methodological implications are addressed separately in the discussion of using two reviewers in a systematic review.

The Bottom Line

Choose a systematic review when answering your question requires explicit, transparent, and sufficiently comprehensive identification and synthesis of eligible evidence; choose a narrative approach when the scholarly purpose is better served by broader conceptual, theoretical, historical, explanatory, or interpretive synthesis.

Do not treat "systematic" as a quality badge or "narrative" as permission to be selective without justification. Define the question first, determine what kind of evidence and inference it requires, and then use the review methodology capable of supporting the claim you intend to make.
